REGISTRATION INFORMATION
Once the camp begins, no refunds will be issued. However, a credit for future camp dates may be granted at the discretion of the Camp Director, with determinations made on a case-by-case basis.
A deposit of $150.00, which will be applied to the total balance, must be submitted along with the registration documents.
Session Pricing per week
Full Day 9:00am-3:00pm $325
Morning Session Only 9:00am-12noon $180
Afternoon session Only 12:30pm-3:00pm $170
The required documents must be completed and submitted to the following address before camp begins:
- Silver Lake Summer Hoops Basketball Camp Registration Form
- Personal Check/ Money Order (Checks should be made payable to: Coach Dick Steele)
- Silver Lake Summer Hoops Medical Consent Form
- Medical Examination Record (Physical Form from Primary Care Physician)
